Community Care Officer

 13 hours per week post -  Lincoln County Hospital base

Plus 15% Attraction & Retention Payment until March 2027 (pro rata)

We're looking for a Community Care Officer (Level 2) to join our Hospital Social Work Team.

Based at Lincoln County Hospital, you'll also support work across the Northern Lincolnshire and Goole NHS Foundation Trust hospital sites, helping people leave hospital safely and regain their independence.

This is a part-time role of 13 hours per week, making it ideal if you're looking for flexibility while still doing meaningful work.


What will you be doing?

No two days are the same, but you can expect to:

✔ Support people and their families to identify what matters most to them

✔ Complete strengths-based assessments and reviews

✔ Arrange care and support that promotes independence

✔ Work alongside social workers, occupational therapists, nurses and ward teams

✔ Help people navigate complex discharge planning

✔ Build strong relationships with health colleagues and community partners

✔ Contribute to safeguarding and risk management where needed

You'll manage your own supervised caseload and play an important role in helping people achieve positive outcomes.
