Continuing Health Care Coordinator

An exciting opportunity has arisen for a permanent, full-time Continuing Healthcare (CHC) Coordinator within the Lead Professional Team.

 If you hold a qualification in Social Work, Occupational Therapy, or Nursing—and have a proven track record of working effectively with partners to apply the NHS Continuing Healthcare Framework—we would love to hear from you

About the Role

In this dynamic role, you will work alongside the current CHC Coordinator, bringing your operational experience and expertise in NHS Continuing Healthcare to further strengthen the team. This is an ideal opportunity to balance direct practice with leadership and development responsibilities.

As CHC Coordinator, you will:

  • Support staff in applying the NHS CHC Framework.
  • Jointly manage complex cases with adult care practitioners.
  • Ensure guidance, training, resources, and practice standards are in place.
  • Monitor CHC activity using appropriate systems.
  • Represent Lincolnshire Adult Care on panels, support dispute resolution, and maintain effective relationships with NHS partners.

About You

We are looking for someone who is ambitious, diligent, and committed to delivering exceptional services to the communities of Lincolnshire.

You will bring:

  • Extensive post-qualification experience in NHS Continuing Healthcare.
  • A strong understanding of adult care legislation and practice.
  • The ability to support staff in fulfilling duties under the Section 75 agreement.
  • Insight into the financial implications of CHC for individuals and adult care.
  • A quality-focused approach to achieving positive outcomes for individuals and the council.
  • Confidence in decision-making, using sound judgment and experience.
  • Proven ability to support staff in managing complex, high-risk cases.
  • A commitment to ensuring adult care perspectives are considered in joint decision-making.

We have embraced new ways of working and hybrid working will be a feature of this role. You will be given a nominated contractual work base and you will be required to commute to this work base when necessary.  Working arrangements regarding hybrid working will be discussed as part of the appointment process.
