We are seeking committed and skilled Learning Disability Nurses/Social Workers as Qualified Practitioners to join our Learning Disability South Team, based at Sleaford/Spalding. 


We have two vacancies: 1 full-time 37 hours and 1 part-time 18.5 hours per week. Salary is dependent on experience.

We are particularly interested to hear from candidates who are Learning Disability Nurses.

About the Role

As a Qualified Practitioner within the Learning Disability South Team, you will play a key role in supporting adults with learning disabilities to live fulfilling, independent, and meaningful lives. You will be responsible for completing Care Act assessments, developing and reviewing support plans, and ensuring that individuals receive the right level of support at the right time.

You will hold a current Nursing or Social Work registration.

You will hold a varied caseload, including more complex cases, and will work with individuals on a long‑term basis, enabling you to build strong, trusting relationships. 

As part of an experienced and enabling team, you will also have opportunities to supervise less experienced colleagues and contribute to learning and development of the team. 

 

Working within Lincolnshire's strengths-based Conversation Model, you will:

  • Complete Adult Needs Assessments and reviews under the Care Act 2014
  • Develop strengths-based support plans that focus on individual outcomes
  • Work alongside individuals, carers, families and partner agencies
  • Promote independence, wellbeing and community connections
  • Respond to safeguarding concerns and contribute to safeguarding enquiries
  • Support people through periods of change, crisis and transition
  • Manage a varied and rewarding caseload across your locality
  • Supervise up to a maximum of 4 staff
  • Leading on section 75 reviews/Decision Support Tool and CHC checklists
  • Support wider Learning Disability teams with nurse rota for section 75 reviews/ Decision Support Tool and CHC checklists
  • The Learning Disability focuses on working with adults who require assessments, Decision Support Tool, care planning, and ongoing interventions to meet their care and support needs. This often involves undertaking comprehensive assessments, developing person-centred support plans, and working collaboratively with families, carers, health professionals, and other partner agencies. The team covers Sleaford, Spalding, Grantham, Stamford and Bourne.

What We’re Looking For

We welcome applications from both newly qualified and experienced Learning Disability Nurses and Social Workers. Experience of working with adults with learning disabilities is desirable but not essential, as full training and support will be provided.

As this is a community‑based role, the ability to travel to complete visits is required.

You will be given a nominated contractual work base and you will be required to commute to this work base when necessary.  Working arrangements regarding hybrid working will be discussed as part of the appointment process.
