Advanced Practitioner Social Work

Job Introduction

Lincolnshire County Council has an exciting opportunity for an Advanced Practitioner position within the Adult Care hospital team. This is an excellent opportunity to work within the hospital discharge system.

The role covers Lincoln and Grantham hospitals, and the West Transfer of Care Team.

Are you an experienced social worker looking for an opportunity to progress for career progression?

You will support the Lead Practitioners in leading reflective supervision, sharing your knowledge and experience with the teams and undertaking complex case work.  

The role is 37 hours per week and is Monday to Friday.

We are looking for an experienced and motivated social worker who is a team player and can offer support and supervision to colleagues as well as carrying a caseload of more complex cases.  Our ethos is to ensure our customers are able to live independent lives and for those that need support, the right support package, in the right place, at the right time.

As a Social Worker you must hold a professional Social Work qualification e.g. DIPSW, CQSW, CETSW, CSS or BA Social Work and ensure that registration with Social Work England is up to date.

You will provide advice, support and guidance to the team.  The role is varied and rewarding.

To take on this exciting role you must hold a professional Social Work qualification and have maintained professional registration with Social Work England
