Community Care Officer - Level 2

Job Introduction

37 hours per week with 15% Attraction & Retention Payment (secured until 31 March 2027)

Are you someone who enjoys working with people, solving problems, and making a real difference to lives every day? Do you want to join a team that works hard, supports each other, and finds time for a laugh along the way?

If that sounds like you, we’d love you to join our Adult Care & Community Wellbeing team at Peterborough City Hospital.

About the team

We’re a close-knit, friendly group of Social Workers, Community Care Officers, and health colleagues who work together to support patients and families during hospital stays and beyond. We know hospital life can feel fast-paced, but in our team you’ll always find support, encouragement, and a good sense of humour to get us through the busy days.

We don’t just work alongside each other – we really work as a team. We share ideas, celebrate wins (big and small), and believe that kindness and collaboration are just as important as professional skills.

What you’ll do

As a Community Care Officer, you’ll:

  • Work with people and families to understand their needs, strengths and goals.
  • Develop care plans and solutions that promote independence and safety.
  • Work closely with Social Workers, therapists, housing and health colleagues to get the best outcomes.
  • Be part of a team that makes sure patients leave hospital with the right support in place.

Join us for:

  • A 15% Attraction & Retention Bonus secured until 31 March 2027 – recognition of the value you bring.
  • A supportive hospital team.
  • Opportunities to learn, grow, and develop your skills with ongoing training.
  • The chance to work in a role that’s challenging, rewarding, and makes a genuine difference.
  • A team that works hard but also knows the importance of good coffee, cake, and celebrating each other’s successes.

Along with a competitive salary we are offering:
- “Green book” Local Government terms and conditions
- Access to Local Government Pension scheme (contribution rate of approx. 24%)
- Comprehensive benefits package including salary sacrifice for cars, cycles, plus excellent discount schemes
- 37 hour working week (as full time) including flexible and hybrid working
- A generous leave entitlement with a minimum of 25 days plus bank holidays and the option to buy more
- Free financial wellbeing advice
